All children under the age of 18 MUST have a Medical Release Form and Waiver of Liability Form completed and signed by their parent/guardian. Please bring these signed forms with you to the conference or scan and email them to the Registrar at registrar@greenlakebahaiconference.net.

If the parent/legal guardian of a minor (under age 18) will not be present with their child at the conference, a Sponsorship Agreement Form MUST be completed and signed by the parent/legal guardian. Please bring this signed form with you to the conference.
